We truly understand how challenging these times are for some, and it’s our heartfelt mission to make healthcare more accessible for everyone. That’s why we are offering such deeply discounted services and flexible payment plans, because we believe that good health should never be out of reach for anyone who wants it.

Our goal is simple:

1. We recognize the financial struggles many face, and we want to ease that burden without compromising on care.
2. We want to promote and aggressively drive preventive care and education, as it’s far more cost-effective to prevent illness than to treat it later.
3. Above all, we want to build lasting, meaningful relationships with our patients and the communities we serve.

If you believe in our mission and help us spread the word by referring others, we want to show our appreciation. As a thank you for your trust and support, we’ll gladly refund one of your monthly payments for the current year. It’s our small way of honoring your belief in what healthcare should truly be.

Please note, the conditions for receiving referral rewards are simple:

1. Referrals must be outside of your immediate family.
2. The referred patient must purchase a plan.

1. This subscription is NOT an insurance plan and is not intended to be a substitute for one. Instead, it’s a prepayment plan designed to offer significant discounts on healthcare services at TLC Women’s Health Center in exchange for increasing preventive care and a long-term relationship with patients. While this plan provides access to affordable services, it does not offer the broad coverage or protections associated with traditional insurance.
2. TLC Women’s Health Center is not a primary care provider and does not function as a primary care office. While we offer a range of health services, our practice is focused on Women’s Health Services, and is not intended to replace traditional primary care services. We recommend that all patients maintain an established relationship with a licensed primary care physician for routine healthcare needs, general medical care, and ongoing health management. By utilizing our services, you acknowledge and agree that our practice is not a primary care provider, and we are not responsible for providing comprehensive primary healthcare services.

Participants in this subscription plan must not have active insurance coverage of their own. This plan is designed for those who are without insurance. If you obtain insurance coverage after subscribing, you are required to notify us and cancel your subscription.

1. A one-time payment in full or monthly automated payments are a requirement to maintain your subscription plan.
2. At least 75% of the annual subscription must be paid before any visits or services can be rendered.
3. Late payments or repeated returned payments may result in termination from the plan, without refund.
4. All services must be purchased via a valid credit or debit card. By providing a credit or debit card, Patient expressly authorizes all charges and/or additional fees to be charged to said payment card, including recurring payments billed on a monthly or annual basis.

1. This agreement, all services and care are provided solely by TLC Women’s Health Center. No care or services will be provided by any other provider, office, or hospital outside of our practice.
2. Appointments are required for all visits, including virtual visits.
3. Two (2) “No Call, No Shows” and/or Two (2) cancellations within 24 hours of your scheduled appointment may result in termination from the plan, without refund.
4. Any unused visits or services do not roll over to the following year.
5. The services provided under this plan cannot be shared or reassigned to other individuals.
6. Any attempt to “game” the system, such as using the plan fraudulently or abusing services, will result in termination of the subscription without refund.

1. If no visits have occurred and no services have been rendered during the given year, a refund of all payments received for given year (minus a 10% processing fee) will be issued upon written request.
2. Once any visits have occurred or any services have been rendered, no refunds will be issued.
3. No refunds will be issued if the termination is due to any of the following:
   Repeated “No Call, No Shows” or multiple cancellations within 24 hours of appointments.
   Having active insurance and failing to disclose and cancelling your subscription.
   Abuse or misuse of services or care.
4. Both subscribers and TLC Women’s Health Center reserve the right to cancel the subscription at any time for any reason, provided it’s done in writing.
5. In order to prevent “gaming the plans”, if a subscriber cancels their subscription before completing the full 12 payments for the year, they must wait 5 years before being eligible to participate in any of the plans again.
6. Subscribers must inform TLC Women’s Health Center if they obtain insurance coverage and request to cancel their subscription.
7. If TLC Women’s Health Center cancels a subscriber’s plan, they may be permanently prohibited from rejoining any of the plans, depending on the reason for cancellation.

1. Changes to Terms: TLC Women’s Health Center reserves the right to amend these terms at any time. Please call our office (954) 900-6207 for the full and latest terms and conditions.
2. Dispute Resolution: Any disputes arising from this agreement will be resolved through binding arbitration rather than court proceedings.
3. Limitation of Liability: To the fullest extent permitted by law, in no event will either party or its affiliates be liable for indirect, incidental, consequential, exemplary, reputational, special, or punitive damages of any kind. The limitations of liability contained in this agreement will survive the termination or expiration of this agreement and plan.
4. Indemnification: The Patient agrees to indemnify and defend TLC Women’s Health Center, its officers, employees, agents, and affiliates, from and against any and all third-party claims, damages, liabilities, or expenses (including reasonable attorneys’ fees) arising out of or in connection with the Patient’s use of the services provided, or any breach of this agreement.
5. Disclaimer of Warranties: Except as specifically set forth in this agreement and to the fullest extent permitted by law, the services are provided “as is” and “as available,” and TLC Women’s Health Center makes no other warranties, express or implied, including but not limited to the implied warranties of merchantability, non-infringement, quiet enjoyment, and fitness for a particular purpose and any warranties arising from a course of dealing or usage in trade. To the extent that TLC Women’s Health Center cannot disclaim any such warranty as a matter of applicable law, the scope and duration of such warranty will be limited to the fullest extent permitted by law.
6. Severability and Waiver: In the event any provision of this Agreement is held by a court of competent jurisdiction to be invalid, void, or unenforceable, such provision(s) will be stricken, and the remainder of this Agreement will remain legal, valid, and binding. The failure by either party to exercise or enforce any right conferred by this Agreement will not be deemed to be a waiver of any such right, nor will it bar the exercise or enforcement of any such or other right on any later occasion. Except as otherwise expressly stated in this Agreement, all rights and remedies stated in this Agreement are cumulative and in addition to any other rights and remedies available under the Agreement, at law, or in equity
